Job summary

Alliance Health is seeking a Care Manager II for a full-time remote position based in North Carolina Cumberland. You will lead care team communication, perform comprehensive assessments, and develop unified plans of care for Tailored Plan recipients while coordinating with healthcare providers.

Qualifications

• Master’s degree in Human Services or related field and active NC licensure as one of the listed credentials.
• Two or more years’ post-graduate MH/SUD experience (or IDD).
• Willing to travel to Alliance offices for meetings as needed.

Responsibilities

• Lead communication among care team and act as primary contact for members.
• Complete comprehensive assessments and develop Plans of Care.
• Assign interventions and plans of care to Care Worker.
• Submit referrals to Integrated Health Consultant for medical/pharmacy needs.
• Coordinate with team to ensure smooth transitions of care and service delivery.
• Document all activities to meet Medicaid requirements and organizational standards.
• Travel locally for meetings and site visits as required.
