Job summary

Alliance Health is seeking a Complex Case Manager to support high-need members in a remote role based in Charlotte, NC. The position requires deep care coordination for individuals with behavioral and physical health conditions and frequent healthcare utilization.

Responsibilities include initial member engagement, assessments, and ongoing care plan development, with travel to meet stakeholders as needed. Strong licensure and certifications are preferred.

Qualifications

• Registered Nurse with valid RN licensure and two years of full-time post-degree care coordination experience in behavioral and physical health.
• Alternative: Master’s degree with two years of care management experience and full licensure (LCSW/LMFT/LCAS/LCMHC/LPA).
• Care management certification preferred.

Responsibilities

• Engage and support members with complex health needs, including multiple chronic conditions and SMI/SUD/I/DD.
• Conduct initial engagement, assessments, and develop individualized care plans with timely follow up.
• Coordinate transitions of care, medication safety, and linkage to community resources.
• Document interactions in Alliance’s electronic care-management system and ensure compliance.
