Job summary

Alliance Health is seeking a Care Manager II to lead communication among the care team and serve as the primary contact for the member. The role includes conducting comprehensive assessments and creating unified care plans for Tailored Plan recipients, with coordination across health service providers.

The position is full-time remote with requirement to reside in North Carolina and willingness to travel to Alliance offices or provider sites for meetings as needed.

Qualifications

• Master’s degree in Human Services and two years post graduate MH/SUD and/or I/DD experience or
• Graduation from a school of nursing and RN license with two years MH/SUD and/or I/DD experience and active license

Responsibilities

• Lead communication among care team members and serve as primary contact for the member
• Complete comprehensive assessments and develop unified plans of care
• Coordinate with providers and community resources to ensure continuity of care
• Document and submit plans of care and required information
• Schedule and participate in team meetings and care conferences
• Provide member education and support about services, rights, and payer requirements
• Travel between Alliance offices and provider sites as needed
