Job summary

Alliance Health is hiring a Care Manager II to lead communication among the care team and serve as the primary contact for Tailored Plan members. You will complete comprehensive assessments and develop unified plans of care, coordinating with providers to meet individual needs.

This full-time remote role requires NC residency with occasional travel to offices for meetings. You will document in JIVA, educate members and LRPs, and collaborate across teams to ensure timely authorizations and

Qualifications

• Master’s degree in Human Services or related field and at least two years of full-time, post graduate degree MH/SUD and/or I/DD experience.
• Or graduation from a school of nursing with RN license and two years MH/SUD and/or I/DD experience.
• Must be licensed in North Carolina for MH/SUD practice.

Responsibilities

• Complete Assessment/Planning for enrollment and ongoing care.
• Provide Support and Monitoring with member education and rights explanations.
• Complete Documentation and coordination of care in JIVA and records.
• Travel between Alliance offices and to meet members/providers as needed.
