Job summary

Alliance Health in Charlotte, NC seeks a Care Manager I – Non-Waiver to coordinate integrated, whole-person care across physical health, behavioral health, pharmacy, and social needs. The role focuses on a specified member population, collaborating with community systems to support outcomes.

The position is full-time and hybrid; reporting onsite for meetings as needed and travel throughout Mecklenburg may be required.

Qualifications

• Bachelor’s degree in Human Services with 2 years mh/dd/sa post-bachelor experience
• Bachelor’s degree in Non-Human Services with 4 years mh/dd/sa post-bachelor experience
• Master’s in Human Services with 1 year mh/dd/sa post-graduate experience
• Fully or provisionally licensed in NC as LCSW/LCMHC/LPA/LMFT
• Licensed RN in NC with four years mh/dd/sa experience
• Preferred certifications NACCM, NADD-Specialist and/or CBIS

Responsibilities

• Complete comprehensive assessments or Care Needs Screening at enrollment, yearly or on condition changes
• Develop Plans of Care from assessments
• Submit referrals to Transition Support Team for medical/pharma complexity
• Assign Plan of Care activities to Transition Support Team for SDOH or complex payer issues
• Assist individuals/legally responsible persons in choosing service providers
• Evaluate service appropriateness and track plan of care implementation
• Collaborate with care team for smooth transitions in care
• Document updates and activities per procedures
• Coordinate travel between Alliance offices as needed
• Attend community/provider meetings for member needs
• Ensure timely authorization and communication with care team
