Job summary

WelbeHealth PACE seeks a Primary Care Provider (NP/PA) to join our team, delivering comprehensive medical services to seniors at home and in communities. You will work with an interdisciplinary IDT to assess patients, plan primary care, and educate participants and families.

This role emphasizes long-term relationships, reduced caseloads, and true value-based care. Compensation includes base pay plus bonus, strong benefits, PTO, 401k match, and a comprehensive package.

Qualifications

Must have nursing degree from an accredited school.
NP or PA with prescriptive authority required.
Minimum 2 years frail/elderly care experience.
Knowledge of chronic care and geriatrics.
Willing to participate in rotating night and weekend on-call.

Responsibilities

Provide continuous medical coverage and in-person assessments (initial, semi-annual, unscheduled).
Collaborate with IDT to develop care plans and coordinate care.
Assist in maintaining participant care in SNFs and residences.
Review outcomes and utilization data to improve practices.
