Job summary

WelbeHealth, LLC. seeks a Primary Care Physician to deliver value-based, holistic care for vulnerable seniors within its PACE program in California.

You will lead comprehensive assessments, coordinate transitions, and operate with a focus on long-term patient relationships, including home visits. The role offers base salary plus bonus, relocation assistance, CME reimbursement, a 401(k) match, health insurance, and a predictable 8am–5pm schedule with occasional on-call.

Qualifications

• Successful completion of an residency program accredited by the ACGME.
• Master's or doctorate Degree.
• Board Certification preferred.
• Active state physician license without restrictions.
• State CDS certification and DEA registration.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR/First Aid certification upon hire preferred.

Responsibilities

• Perform comprehensive in-person assessments with the interdisciplinary team to develop a personalized care plan.
• Coordinate discharge for acute, short-term, and long-term placement.
• No hospital rounds; focus on comprehensive care, seeing an average of 8 patients a day.
