Job summary

WelbeHealth, a PACE program, seeks a Primary Care Provider (Nurse Practitioner or Physician Assistant) in Newark, NJ to manage primary care for elderly participants within a value-based care framework. You will work with WelbeHealth physicians and the IDT to assess, plan, and educate participants and families.

The role offers a smaller patient caseload for meaningful relationships, collaboration with IDT, and a competitive compensation package with bonus, comprehensive benefits, and 401(k) match.

Qualifications

• Graduation from an accredited school of nursing.
• Certificate as a Nurse Practitioner or Physician Assistant with prescriptive authority.
• Minimum of two years of experience working with frail or elderly populations.
• Current knowledge of chronic care and geriatric issues.
• Willingness to participate in rotating night and weekend call.

Responsibilities

• Provide continuous medical coverage and conduct comprehensive in-person assessments (initial, semi-annual, and unscheduled).
• Collaborate with Physicians and IDT to develop participant care plans and coordinate care.
• Assist in maintaining participant care in skilled nursing facilities and residences with regular visits.
• Review outcomes and utilization data to establish best practices and policy improvements.
