WelbeHealth, a Leading PACE organization in Fresno, CA, seeks a Primary Care Provider to deliver comprehensive medical assessments and care planning within an IDT. You will coordinate with physicians, nurses, and therapists to maintain high-quality, person-centered care in home and facility settings.

The role emphasizes a small patient caseload, value-based care, and collaboration. Must hold NP/PA credentials with prescriptive authority and a valid RN license.

Qualifikationen

• RN license is required.
• Graduation from an accredited school of nursing is required.
• Nurse Practitioner or Physician Assistant with prescriptive authority is required.

• Provide continuous medical coverage to participants with in-person assessments (initial, semi-annual, unscheduled).
• Develop participant care plans with the IDT and coordinate care across settings.
• Assist in discharge planning from acute or long-term care and perform regular visits in facilities and homes.
