WelbeHealth PACE, Fresno, CA, seeks a Primary Care Provider (NP/PA) to deliver comprehensive care to seniors in home and community settings. You will assess participants, plan primary care, and educate families under Medical Director direction.

The role emphasizes smaller caseloads for long-term relationships, collaboration with IDT, and a commitment to value-based care with a competitive compensation package including bonus and benefits.

Qualifikationen

• Graduation from an accredited school of nursing.
• Certificate as a Nurse Practitioner or Physician Assistant with prescriptive authority.
• Minimum of two (2) years of experience working with frail or elderly populations.
• Current knowledge of chronic care, geriatric issues, and best practices.
• Reliable means of transportation.
• Able and willing to take part in rotating night and weekend call.

Aufgaben

• Provide continuous medical coverage to participants, including comprehensive in-person assessments (initial, semi-annual, unscheduled).
• Collaborate with Physicians and IDT to develop participant care plans and ensure effective care coordination.
• Assist in maintaining participant care in skilled nursing facilities and residences with regular visits as dictated by participant need.
• Review outcomes and utilization data to establish best practices and policy improvements.
