Job summary

WelbeHealth is hiring a Primary Care Provider, Nurse Practitioner or Physician Assistant, to collaborate with an interdisciplinary team and oversee comprehensive assessments for participants living in their homes and communities.

You will develop care plans with WelbeHealth physicians, coordinate care, and ensure high-quality outcomes for a senior, frail population in a value-based care model.

Qualifications

• Must have graduated from an accredited school of nursing.
• Certificate as a Nurse Practitioner or Physician Assistant with prescriptive authority.
• Minimum of two years of experience working with frail or elderly populations.
• Knowledge of chronic care, geriatric issues, and best practices.
• Reliable transportation and ability to participate in rotating night/weekend call.

Responsibilities

• Provide continuous medical coverage to participants and perform comprehensive assessments (initial, semi-annual, unscheduled).
• Develop participant care plans with Physicians and IDT and coordinate care transitions/outcomes.
• Maintain participant care in skilled nursing facilities and residences with regular visits as needed.
• Review outcomes and utilization data to inform best practices and policy improvements.
