Job summary
WelbeHealth is seeking a Primary Care Provider (Nurse Practitioner or Physician Assistant) in San Jose, CA to deliver value-based care and partner with an interdisciplinary team. You will conduct comprehensive assessments, develop care plans with physicians, and coordinate ongoing care for frail seniors in their homes and facilities.
Role requires geriatric expertise, prescriptive authority, and willingness to engage in rotating night and weekend call.
Qualifications
- Must be a graduate of an accredited nursing program and licensed as a Nurse Practitioner or Physician Assistant with prescriptive authority.
- Strong knowledge of geriatric and chronic care best practices; experience with frail/elderly populations preferred.
- Ability to collaborate with the IDT and provide comprehensive primary care assessments.
Responsibilities
- Provide continuous medical coverage and conduct comprehensive in-person assessments (initial, semi-annual, unscheduled).
- Develop participant care plans with Physicians and IDT; coordinate care and discharge planning.
- Perform regular visits to participants in skilled nursing facilities and residences as needed.
- Review outcomes and utilization data to improve practices and policy.
