Job summary

WelbeHealth, LLC. is seeking Primary Care Physicians for a value-based PACE program. You will perform comprehensive in-person assessments with the interdisciplinary team to develop personalized care plans for Medicare and Medi-Cal participants.

We offer a competitive total rewards package, relocation assistance, CME reimbursement, malpractice coverage, and a growth path toward medical director roles. Regular hours are 8am–5pm, Monday through Friday, with some home visits and rotating on-call.

Qualifications

• Must complete residency program accredited by the ACGME.
• MD or DO degree required; Board certification preferred.
• Active state physician license without restrictions required.
• State CDS certification and DEA registration.
• CPR and First Aid certification within 180 days of hire preferred.

Responsibilities

• Perform comprehensive in-person assessments with IDT to develop personalized care plans.
• Coordinate discharge for acute, short-term, and long-term placement.
• Focus on comprehensive care with about 8 patient visits per day; no hospital rounds.
