Job summary

ChenMed is seeking a Patient Continuity and Needs Advocate (PCNA) to bridge patients with comprehensive care and tailored health plans. You will collaborate with center leadership, sales teams, brokers and clinical staff to retain patients and ensure seamless care transitions.

The role requires strong communication, HIPAA compliance, and experience with EMR/Salesforce. A high school diploma is required, with a BA/BS preferred and at least 3 years of experience, especially with seniors.

Qualifications

• High School diploma or GED required.
• BA/BS degree preferred or equivalent experience considered.
• Minimum 3 years’ work experience; senior care experience a plus.

Responsibilities

• Advocate for patients and connect them with health plans and community resources.
• Proactively reach out to disenrolled members to re-engage care.
• Collaborate with center leadership, sales teams, brokers and clinical staff to retain patients.
• Ensure seamless care transitions and prevent gaps in care.
• Document patient issues in EMR, Medals and Salesforce; maintain work queue.
• Comply with HIPAA and other regulatory requirements.
• Perform other duties as assigned.
