Job summary

ChenMed seeks a Patient Continuity and Needs Advocate (PCNA) to bridge patients with comprehensive care and support services. The role emphasizes strong patient relationships, care coordination, and proactive outreach to ensure eligible patients access appropriate plans and services.

The PCNA will work with center leadership, brokers, and clinical staff to minimize care disruptions, document patient issues in Salesforce/EMR, and ensure HIPA A compliance while promoting compassionate,

Qualifications

• High School diploma or GED required.
• BA/BS degree preferred or equivalent experience considered.
• Minimum 3 years of work experience; Medicare Advantage knowledge is a plus.

Responsibilities

• Advocates for patients and connects eligible individuals with health plans and community resources.
• Proactively reaches out to disenrolled members to understand barriers to care and facilitates their return.
• Collaborates with center leadership, sales teams, brokers and clinical staff on patient retention strategies.
• Ensures seamless care transitions and sustained engagement to prevent gaps in care.
• Documents patient interactions in EMR and Salesforce; maintains assigned work queue.
• Complies with HIPAA and other regulatory requirements.
