Job summary

ChenMed LLC is hiring a Patient Continuity and Needs Advocate (PCNA) to bridge patients with tailored healthcare plans and support services. You will maintain strong patient-provider relationships and help ensure patients receive comprehensive primary care and entitlements.

The role emphasizes care continuity, documentation in EMR/Salesforce, and collaboration with medical centers to minimize care disruptions for seniors.

Qualifications

• High School diploma or GED required.
• BA/BS degree preferred or additional experience considered in lieu of education.
• Minimum of three (3) years’ work experience; working with seniors a plus; Medicare Advantage knowledge is a plus.

Responsibilities

• Advocates for patients and connects eligible individuals with health plans and community resources.
• Proactively reaches out to disenrolled members to understand and address barriers to care and reunite them with care services.
• Collaborates with center leadership, sales teams, brokers and clinical staff to develop patient retention strategies.
