Job summary

Qualifications

• High School diploma or GED required.
• BA/BS degree preferred or equivalent experience considered.
• Minimum of three years' work experience; Medicare Advantage knowledge a plus.

Responsibilities

• Advocates for patients and connects them with health plans and community resources.
• Proactively re-engages disenrolled members to restore care continuity.
• Collaborates with center leadership, sales brokers, and clinical staff on retention strategies.
• Ensures seamless transitions and reduces gaps in care.
• Documents patient interactions in EMR and Salesforce while maintaining HIPAA compliance.
