Job summary

ChenMed is seeking a Patient Continuity and Needs Advocate (PCNA) to bridge patients with comprehensive care. You will support seniors by linking eligible individuals to health plans, community resources, and ongoing services.

Qualifications

• High School diploma or GED, required.
• BA/BS degree preferred or equivalent experience.
• Minimum of three years’ work experience; Medicare Advantage knowledge a plus.

Responsibilities

• Advocate for patients and connect eligible individuals with health plans and resources.
• Proactively reach out to disenrolled members to facilitate their return to care.
• Partner with center leadership, brokers, and clinical staff on retention strategies.
• Ensure seamless care transitions and sustained engagement.
• Document patient issues in EMR, Medals and Salesforce; manage queue.
• Ensure HIPAA-compliant documentation.
• Performs other duties as assigned.
