Job summary

ChenMed is seeking a Patient Continuity and Needs Advocate (PCNA) to bridge patients with comprehensive care and tailor support services to individual needs. You will work to ensure patients receive the right health plans and benefits while supporting transitions and ongoing engagement at ChenMed centers.

The role emphasizes patient relationships, HIPAA compliance, and collaboration with clinicians, brokers, and leadership to reduce disruptions and improve outcomes for seniors in ChenMed’s

Qualifications

• High School diploma or GED required
• BA/BS degree preferred or equivalent work experience considered
• Minimum of three years’ work experience, preferably with seniors and Medicare Advantage knowledge

Responsibilities

• Advocate for patients by connecting eligible individuals with health plans and community resources.
• Proactively reach out to disenrolled members to understand barriers and facilitate return to care.
• Collaborate with center leadership, sales teams, brokers and clinical staff on retention strategies.
• Ensure care continuity and prevent gaps in care that could lead to hospital visits.
• Document patient issues and resolutions securely in EMR systems (Medials and Salesforce) and maintain work queues.
• Comply with HIPAA and other regulatory requirements; perform additional duties as assigned.
