Job summary

ChenMed seeks a Patient Continuity and Needs Advocate (PCNA) to act as a crucial link between patients and comprehensive care. You will help eligible patients access health plans and support services, building trust and improving care continuity in our senior-focused model.

In this role, you will collaborate with center leadership, brokers, and clinicians to reduce barriers to care, while ensuring accurate, HIPAA-compliant documentation across EMR systems and Salesforce.

Qualifications

• High School diploma or GED required.
• BA/BS preferred OR equivalent experience considered year-for-year.
• Minimum of three years of work with seniors; Medicare Advantage knowledge a plus.

Responsibilities

• Advocate for patients and connect eligible individuals with health plans and community resources.
• Retain and re-engage disenrolled members through personalized outreach and support.
• Collaborate with center leadership, sales teams, brokers and clinical staff to develop patient retention strategies.
• Ensure care continuity and prevent gaps that could lead to hospital visits.
• Document patient information in EMR, Medals and Salesforce; maintain assigned work queue and respond to questions.
• Comply with HIPAA and regulatory requirements in all patient documentation.
• Perform other duties as assigned by management.
