Qualifications

• Proven ability to communicate clearly with clients, families, and caregivers.
• Experience leading teams or coordinating care in fast-paced settings.
• Ability to solve problems and stay organized under pressure.
• 2+ years in home or community care preferred.
• Reliable transportation to service areas.

Responsibilities

• Manage, support, and communicate with caregivers within assigned locations using a community database.
• Serve as a problem-solver for clients and caregivers, addressing concerns proactively.
• Collaborate with families and facility staff to ensure high-quality care delivery.
• Supervise caregivers, including coaching and disciplinary actions when necessary.
• Conduct supervisory client visits in compliance with state regulations.
• Coordinate caregiver schedules and client assignments with real-time adjustments.
• Monitor compliance with company and facility policies and procedures.
• Ensure care plans are executed and patient records stay up to date.
• Assess client and family needs and respond with sound decisions.
• Assign caregivers while maintaining cost efficiency.
• Manage intake coordination for new client referrals.
• Review timesheets and documents weekly for accuracy.
• Perform administrative duties and participate in staff training.
