Synergy Home Care in Northeast Chicago, IL is seeking a versatile Care Services Coordinator & Community Liaison to support field care management and strengthen referral networks with hospitals, case managers, and senior communities.

This role blends on-site supervision with community outreach and requires participation in after-hours on-call rotation to ensure continuous caregiver support and client satisfaction.

Qualifications

• Experience in home care or senior services preferred.
• CNA/LPN certification a plus.
• Proven supervision and/or marketing/outreach experience.
• Excellent communication and organization skills.

Responsibilities

• Oversee client care management performance and quality of care.
• Build and maintain referral relationships with hospitals, discharge planners, physicians, and senior communities.
• Represent Synergy in the community at events and health fairs.
