Job summary

ArchWell Health is seeking a Nurse Care Manager to coordinate and evaluate care for patients with acute and chronic conditions, across the care continuum. This role focuses on developing care plans with the care team and executing them to achieve goals, ensure regulatory compliance, and deliver high-quality patient care.

You will work with the care team across centers, acute facilities, and home settings to support transitions of care and optimize outcomes for seniors in value-based care

Qualifications

• RN licensure in state of employment is required.
• Minimum 2 years clinical work experience.
• 1 year of utilization review, home health, discharge planning experience desired.
• 1 year of case management in acute or ambulatory settings desired.
• Certified Case Manager certification preferred or willingness to obtain.

Responsibilities

• Establish a trusting relationship with patients, families, and caregivers.
• Collaborate with clinical staff to achieve patient goals.
• Reduce network costs by driving referrals to preferred groups.
• Identify high-risk/complex patients and develop individualized care plans.
• Perform case management following CMSA standards of practice.
• Provide patient education and care coordination to help patients achieve health goals.
• Communicate with hospital case managers, PTs, social workers, patients, and families to facilitate discharge planning.
• Manage transitions of care and post-discharge coordination.
