ArchWell Health is seeking a Nurse Care Manager to coordinate and evaluate care for patients with acute and chronic conditions across the care continuum, from centers to hospital and home environments.

You will develop comprehensive care plans with the care team and execute them to meet quality standards, regulatory compliance, and positive patient outcomes, while working in a collaborative, patient-centered model.

Qualifications

• Associate degree in Nursing required.
• BSN preferred or RN with related clinical degree.
• Active RN license in state of employment, willingness to obtain elsewhere.
• Minimum 2 years clinical work experience.
• 1 year in utilization review/home health/discharge planning desired.
• 1 year in case management in acute or ambulatory settings desired.
• Certified Case Manager preferred; CCMC/CMCN or willingness to obtain.
• ICD-10/CPT knowledge preferred; managed care experience preferred.
• Fluency in Spanish or other needed languages.
• Mission-driven and patient-centered.

Responsabilités

• Establish trusting relationships with patients and families.
• Collaborate with care team to achieve patient goals.
• Develop comprehensive, individualized care plans.
• Perform case management following CMSA standards.
• Provide patient education and care coordination.
• Support transitions of care and discharge planning.
• Communicate with hospital case managers, PTs, social workers, and families to facilitate safe discharge.
• Monitor progress toward goal achievement.
