Job summary

ArchWell Health is seeking a Nurse Care Manager to coordinate and evaluate care for patients with acute and chronic conditions across the care continuum. You will develop comprehensive care plans with the care team and execute them to meet performance standards, regulatory compliance, and high-quality patient care.

You will work closely with hospital case managers, PTs, social workers, patients, and families to ensure safe transitions of care, discharge planning, and effective home care

Qualifications

• Associate degree in Nursing required.
• BSN preferred; RN with bachelor’s degree in related field acceptable.
• Active, unrestricted RN license in the state of employment required.
• Willingness to obtain RN license in other states with ArchWell Health centers.

Responsibilities

• Establish trusting relationships with patients and families.
• Collaborate with clinical staff to achieve patient goals.
• Identify high-risk/complex patients and develop care plans.
• Perform case management following CMSA standards of practice.
