Job summary

RN license in the employment state is required, with BSN preferred. Minimum 2 years clinical experience and 1 year in utilization review or discharge planning is highly desired.

Qualifications

• RN license in state of employment required.
• Bachelor's degree in nursing preferred.
• Minimum 2 years clinical experience required.
• Minimum 1 year utilization review/home health/discharge planning experience highly desired.
• Certification in Case Management preferred; willingness to obtain within specified time.
• Fluency in Spanish or other languages helpful in communities served.
• Managed care knowledge including HEDIS and CMS guidelines preferred.

Responsibilities

• Coordinate and evaluate management of patients with acute and chronic conditions.
