Job summary

ArchWell Health in Jacksonville, FL is seeking a Nurse Care Manager to coordinate and evaluate management of patients with acute and chronic conditions across the care continuum, from our centers to post-acute facilities and home environments.

You will develop individualized care plans with the care team, ensure regulatory compliance and quality patient care, and support transitions of care with telephonic and in-person outreach as needed.

Qualifications

• Associate degree in Nursing is required.
• RN license in Florida is required and unrestricted.
• BSN preferred and relevant clinical experience valued.

Responsibilities

• Establish trust with patients, families, and caregivers.
• Collaborate with clinical staff to meet patient goals.
• Assess needs and develop comprehensive care plans.
• Coordinate transitions of care from facilities to home.
• Provide patient education and care coordination interventions.
• Communicate with hospital case managers, PTs, social workers, and families
• Perform case management following CMSA standards.
