Qualifications

• RN licensure in the state of employment
• Associate degree in Nursing required
• Bachelor’s degree in Nursing or RN with related field preferred
• Minimum 2 years of clinical work experience
• 1 year of utilization review, home health, discharge planning experience desired
• 1 year of case management experience in acute or ambulatory settings preferred
• Certified Case Manager certification preferred or willingness to obtain
• ICD-10, CPT, HCPCS knowledge preferred
• Managed care experience with knowledge of HEDIS and CMS guidelines
• Bilingual abilities beneficial where applicable

Responsibilities

• Establish trusting relationships with patients, families, and caregivers
• Collaborate with clinical staff to achieve patient goals
• Drive referrals to preferred groups to reduce network costs
• Identify high-risk/complex patients and develop comprehensive care plans
• Perform case management and follow nursing process and CMSA standards
• Provide patient education and care coordination to achieve health goals
• Use Motivational Interviewing to engage patients
• Coordinate transitions of care from centers to home or post-acute settings
• Communicate with hospital case managers, PTs, social workers and families to facilitate safe discharges
• Manage and plan transitions of care and discharge planning
