Job summary

WellMed, part of the Optum family, is seeking a Healthcare Manager to support high-risk patients in navigating the healthcare system. You will educate, advocate, and collaborate with nurses, physicians, and other care team members to improve quality of life.

In this role, you will perform nursing duties within RN scope, coordinate care plans, and contribute to education and transitions of care. A patient-centered approach and strong communication are essential.

Qualifications

• RN with active, unrestricted license in state of employment
• Current BLS certification required
• 2+ years in physician's office, clinical, or hospital setting
• Knowledge of chronic diseases (COPD/diabetes/CHF/IHD)
• Proficient EMR computer skills
• Strong verbal and written communication
• Organizational and prioritization skills
• Ability to work with multidisciplinary teams
• Tuberculosis screening and immunity proof required

Responsibilities

• Identify patients at high risk with providers and clinic staff
• Support longitudinal care: assess health conditions, reconcile medications, motivational interviewing, patient education, referrals
• Support transitions from inpatient to outpatient care: assess transitional needs, contingency planning, post-discharge assistance
• Coordinate with providers to update care plans
• Document accurately in EMR
• Participate in daily huddles and PCC meetings
• Prepare reports for weekly meetings
• Maintain current nursing competencies
• Perform other related duties as assigned
