Innovative Integrated Health is seeking an Intake Specialist to manage enrollment forms, coordinate site visits, and verify Medi-Cal and Medicare coverage for prospective participants. You will update tracking logs, support with pre-initial assessments, and align processes with CMS and DHCS policies.

The role requires strong communication, attention to detail, and ability to work with an interdisciplinary team to ensure timely enrollment and participant satisfaction.

Qualifications

• Proficient in MS Office suite (Word, Excel, Outlook).
• Strong written and verbal communication skills.
• Ability to learn department policies and services quickly.
• Attention to detail and organizational skills.
• Customer-focused with flexible prioritization.

Responsibilities

• Answer calls from prospective enrollees and provide information on eligibility and services.
• Schedule intake meetings and site tours for prospective enrollees.
• Complete intake assessments and verify Medi-Cal and Medicare coverage.
• Update enrollment data in systems and coordinate Level of Care assessments.
• Maintain intake logs and support enrollment and marketing activities.
