Qualifications

• Requires High School Diploma or GED.
• Experience with frail or elderly populations is preferred.
• Knowledge of CMS/DHCS and PACE policies is beneficial.

Responsibilities

• Answer calls from prospective enrollees and provide information on eligibility and services.
• Schedule face-to-face meetings and site tours with prospective enrollees.
• Complete intake assessments and verify Medi-Cal and Medicare coverage.
• Update enrollment data in PrimeSuite and PaceLogic.
• Maintain intake files and participate in enrollment reporting.
