Responsibilities

• Plans and manages complex care coordination cases to support timely patient access and continuity of care.
• Coordinates with providers, pharmacies, payers, and internal partners to resolve barriers and align next steps.
• Analyzes case trends to identify root causes and improve processes.
• Guides less experienced team members on complex case handling and documentation expectations.
