Presbyterian Healthcare Services is seeking a dedicated Care Coordinator/Case Manager to facilitate interdisciplinary collaboration and coordinate patient care across hospital, clinic, and home settings. The role emphasizes cost-effective, quality outcomes and comprehensive care planning.

The position requires an Associates Degree in Nursing with CCM certification and offers a full-time schedule with Monday-Friday shifts. On-site support and extensive benefits are provided.

Qualifications

Associates Degree in Nursing; 3 years additional experience can substitute for an Associates Degree.
Bachelor's Degree preferred with 1 year of experience.
CCM-Certified Case Manager.

Responsibilities

Supports patients in a hospital, inpatient or clinic setting.
Facilitates a team approach to ensure interventions and cost-effective, quality care.
Conducts in-depth health risk assessments and develops care plans.
Monitors and adjusts plans to improve health outcomes.
Educates providers, staff, members and families on care coordination.
