Qualifications

• Associates Degree in Nursing required; 3 years of additional experience can substitute.
• Bachelor’s Degree and 1 year of experience preferred.
• 3 years of related experience required; CCM-Certified Case Manager preferred.

Responsibilities

• Supports patients in hospital and clinic settings.
• Facilitates team-based care planning and coordination across care settings.
• Conducts health risk and needs assessments to develop and communicate plans for services.
• Monitors and adjusts care plans to improve health outcomes.
• Educates providers and families on care coordination goals and processes.
