Job summary

Extended Care Specialists INC is seeking a Care Coordinator to manage patient caseloads, coordinate primary and specialty care, and oversee transitions of care in a skilled nursing/assisted living setting.

You will collaborate with patient schedulers and interdisciplinary teams, ensure accurate documentation, and support Medicare CCM/BHI initiatives while maintaining patient confidentiality and high-quality care goals.

Qualifications

• LPN degree or certified MA with 5 years primary care experience.
• 2 years minimum experience in the LTC/ALF setting.
• Care coordination experience preferred.
• Value based care experience ideal.
• Strong understanding of cultural competency with the target population.
• Competency with Excel, PCC, Matrix and Microsoft products.

Responsibilities

• Manage client caseloads and coordinate primary and specialty care.
• Oversee transitions of care and follow-up appointments.
• Communicate with patient families and care team post visits.
• Document services in patient records and ensure labs and notes are available.
• Refer patients to needed services and schedule follow-ups within 1 week after discharge.
• Track time spent on care coordination for CCM minutes.
• Ensure Medicare CCM and BHI program compliance.
