Job summary

HSCSN is seeking an RN Care Manager to provide comprehensive care coordination for enrollees with high complexity medical and behavioral needs. You will manage a caseload, develop and monitor treatment plans, and coordinate services with PCPs and multidisciplinary teams in a hybrid field office setting.

Qualified candidates hold an associate degree in nursing (bachelor preferred), with at least 2 years in care management and 1 year in public health.

Qualifications

Must have an advanced knowledge of clinical standards of care and disease processes.
Excellent verbal and written communication skills, with telephonic and keyboarding abilities.
Spanish Speaking preferred.
Proficient in web-based technology and MS Office applications.
Ability to meet deadlines and handle multiple demands with service excellence.
Understand confidentiality policies and HSCSN guidelines.
Experience in fast-paced health care environments.

Responsibilities

Manages a caseload as outlined by leadership and care management.
Maintains compliance with contractual and care management requirements.
Conducts outreach and face-to-face visits in homes or offices as needed.
Develops and updates individualized care plans with PCP and multidisciplinary team.
Documents enrollee findings timely and accurately.
Participates in discharge planning and transitions of care.
Educates enrollees on medications and self-management within scope.
Refers to vendors for DME and educational resources.
