Job summary

ChenMed is seeking a Patient Continuity and Needs Advocate (PCNA) in California to bridge patients with suitable healthcare plans and services, ensuring continuous, comprehensive primary care for our senior populations.

The PCNA will work with center leadership, brokers, and clinical staff to maximize patient engagement, minimize care disruptions, and document patient interactions using EMR and Salesforce, all while maintaining HIPAA compliance.

Qualifications

• High School diploma or GED required; BA/BS preferred or equivalent experience.
• 3+ years of experience, preferably with seniors and Medicare Advantage.
• Experience building trust with patients, caregivers and partners; strong care-coordination focus.

Responsibilities

• Advocate for patients and connect eligible individuals with health plans and community resources.
• Proactively reach out to disenrolled members to understand barriers and re-engage them.
• Collaborate with center leadership, sales teams, brokers and clinical staff on retention strategies.
• Ensure care continuity and prevent gaps that could lead to hospital visits.
• Document patient issues in EMR and Salesforce; maintain work queues and ensure HIPAA compliance.
