Job summary

ChenMed LLC is seeking a Patient Continuity and Needs Advocate to bridge patients with appropriate healthcare plans and support services. You will work to ensure continuous, tailored primary care and reduce care disruptions for our senior patients.

In this role, you will advocate for patients, coordinate with centers, brokers, and clinicians, and focus on re-engagement and seamless transitions of care while maintaining HIPAA-compliant documentation and high-quality customer service.

Qualifications

• High school diploma or GED required.
• BA/BS degree preferred OR additional experience considered in lieu of education.
• Minimum of three years’ work experience; Medicare Advantage knowledge a plus.

Responsibilities

• Advocate for patients and connect eligible individuals with health plans and community resources.
• Reach out to disenrolled members to address barriers and re-engage them with care.
• Collaborate with center leadership, brokers, and clinical staff on retention strategies.
