Aufgaben

• Maintain an ongoing caseload of approximately 65 clients and meet NYC aging requirements.
• Conduct comprehensive assessments, develop care plans, and coordinate services with clients and families.
• Process and review necessary paperwork for benefits and services.
• Maintain detailed client records and case notes in the online platform.
• Participate in weekly staff meetings and case conferences as needed.
• Provide crisis intervention and link clients to community resources.
