Job summary

Dot Cares, LLC is seeking an experienced Care Manager to oversee client care across Manhattan. You will lead assessments, build care plans, and advocate with families and providers, taking ownership of each case while collaborating with a support team.

The role is field-based with in-person visits and some remote coordination, offering flexible scheduling. This position is ideal for an RN or social worker who thrives on complex cases and independent ownership.

Qualifications

  • Active NY State license as RN or Licensed Social Worker (LSW, LMSW, or LCSW)
  • Experience in care management, case management, home health, hospice, hospital discharge planning, geriatrics, or social work
  • Strong communication, organization, and relationship-building skills
  • Ability to work independently across homes, medical offices, hospitals, and facilities
  • Ability to travel reliably throughout Manhattan
  • Professional liability/malpractice insurance appropriate to your license
  • Strong computer and documentation skills
  • BSN (nursing) or MSW (social work) preferred
  • Experience with older adults and complex medical/psychosocial needs
  • Experience as a lead/primary case manager
  • Aging-in-place/geriatric care management experience
  • Familiarity with home care agencies, assisted living, skilled nursing, rehab, hospice
  • Experience coordinating complex hospital discharges

Dot Cares is a client-centered aging life care and care management practice serving families throughout New York City, the Lower Hudson Valley, and Southern Connecticut. We help older adults and their families navigate the medical, social, logistical, and emotional complexities of aging, chronic illness, and major life transitions — serving as trusted advocates who coordinate resources and make sure nothing falls through the cracks.

About the Role

We're seeking an experienced Care Manager for clients throughout Manhattan. This is a hands-on lead role: you'll be the primary point of contact for assigned clients, families, and providers, driving the day-to-day work of each case — conducting assessments, developing care plans, attending medical appointments, advocating for clients, coordinating providers, communicating with families, and ensuring follow-through. A collaborative team is available for consultation and coverage, but this role requires someone comfortable owning their cases independently.

Great fit for an experienced RN or social worker who enjoys building relationships, solving complex problems, and taking real ownership of their work.

Key Responsibilities
  • Assessment & Care Planning: Conduct comprehensive assessments (medical, functional, cognitive, emotional, psychosocial, environmental); develop and update individualized care plans; identify and proactively address risks.
  • Medical Advocacy: Attend appointments with clients when appropriate; advocate for their needs; help prepare questions; document and track referrals, testing, and follow-ups.
  • Care Coordination: Coordinate communication among physicians, specialists, therapists, agencies, and facilities; manage transitions between care settings; support discharge planning and close gaps in care.
  • Family Communication: Serve as a trusted point of contact; provide timely updates; help families navigate decisions on safety, independence, and living arrangements.
  • Aging in Place & Safety: Monitor client functioning and environment; identify concerns (falls, medication issues, isolation, caregiver strain); coordinate home care, transportation, equipment, and community resources.
  • Documentation: Maintain accurate, timely case notes and organized files; track outstanding tasks and deadlines; maintain confidentiality and professional boundaries.
What We're Looking For

Proactive, highly organized, and comfortable owning complex cases. A strong communicator with families and healthcare professionals. Compassionate but boundaried, resourceful in unfamiliar situations, able to juggle multiple clients, and diligent about documentation and follow-up.

Compensation & Growth

Starting at $70/hour. Part-time, 1099 to start, with flexible scheduling and strong potential to grow into a full-time W-2 role as caseload expands. Collaborative team environment with consultation and coverage support.
