We help older adults and their families navigate the medical, social, logistical, and emotional complexities of aging, chronic illness, and major life transitions — serving as trusted advocates who coordinate resources and make sure nothing falls through the cracks.
We're seeking an experienced Care Manager for clients throughout Manhattan. This is a hands-on lead role: you'll be the primary point of contact for assigned clients, families, and providers, driving the day-to-day work of each case — conducting assessments, developing care plans, attending medical appointments, advocating for clients, coordinating providers, communicating with families, and ensuring follow-through. A collaborative team is available for consultation and coverage, but this role requires someone comfortable owning their cases independently.
