Qualifications

• Bachelor’s or graduate degree in social work or related health/human services field required.
• Supervised field experience in case management or two years of direct clinical care required or preferred.

Responsibilities

• Build trusting, person-centered relationships with individuals and families.
• Coordinate and monitor services across providers and community resources.
• Translate goals into meaningful supports and services.
• Maintain accurate, timely documentation of activities per requirements.
• Travel within the assigned service region and participate in supervision and professional development.
