Job summary

Capital District Physicians Health Plan Inc in New York seeks an experienced Case Manager to conduct program activities in line with NYS DOH, CMS, NCQA standards. You will identify members, educate, support, coach, and coordinate care with providers and community resources to optimize outcomes.

The role requires collaboration with interdisciplinary teams to create care plans, assess needs, and monitor progress.

Qualifications

• Associates degree required.
• Bachelor's degree preferred.
• Active NYS RN or RD or PT licensure required.
• Minimum of three years of clinical experience; CM experience preferred.
• Proficiency with Microsoft Office Suite.
• Experience interpreting managed care benefit plans and government contracts (Medicare/Medicaid) preferred.
• Strong written and verbal communication; ability to multitask.

Responsibilities

• Handle Level I–III case management activities with appropriate standards.
• Collaborate with providers and community resources to develop care plans.
• Coordinate services and assess member education and support needs.
• Maintain confidentiality and comply with regulatory requirements.
• Participate in cross-functional, multi-disciplinary teams to improve outcomes.
