Qualifications

Responsibilities

• Maintain an ongoing caseload of 65 clients and contact as per NYC Aging requirements.
• Conduct comprehensive assessments, develop care plans, coordinate entitlements and referrals.
• Complete approximately 1,500 case management hours per year.
• Maintain compliance with NYC Aging regulations and documentation.
• Utilize online platform to maintain client records and assessments.
• Participate in meetings, trainings and case conferences.
