Matter of Care in New York, NY is seeking a Clinical Assessment Manager to conduct in-home assessments, evaluate eligibility for community-based services, and develop individualized care plans that promote independence for older adults.

The role includes hands-on clinical tasks as needed, collaboration with care teams, and documentation in EMR systems. A strong nursing background and travel within service areas are required.

Qualifications

• Current, active NYS Registered Nurse license.
• Minimum 5 years of clinical nursing experience.
• Experience in managed care, PACE, MLTC, home care, case management, utilization management, population health, or related settings.
• Strong clinical judgement, communication, and documentation skills.
• Ability to travel throughout service areas.
• BSN preferred.
• Experience utilizing UAS-NY assessments and knowledge of PACE/Community-Based Long-Term Care programs.

Responsabilités

• Conduct in-home clinical assessments using the UAS-NY.
• Evaluate participant eligibility and ongoing care needs for community-based long-term care services.
• Perform reassessments and identify changes in condition, functional status, diagnoses, and medications.
• Develop proposed service plans and recommendations to support participant health and independence.
• Provide hands-on services in participants' homes as needed within scope of practice.
• Document clinical findings in electronic medical records.
• Collaborate with Enrollment, Intake, Care Delivery, and IDT teams; participate in quality initiatives.
