Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read technical procedures.

Responsabilités

• Plan and conduct individual, small and large group activities.
• Assist in communication between staff, residents, families, support personnel, government agencies and the public.
• Develop monthly activity calendars and maintain attendance records.
• Assist in discharge planning and development of activity care plans and resident assessments.
• Arrange transportation for residents for outings, appointments, or discharge.
