Job summary

PACS is seeking an Activity Assistant to support the Activity Director in planning and conducting individual, small and group activities that meet residents' physical, mental, and psychosocial needs.

You will assist with calendars, attendance, transportation, and communication with residents, families, staff, and community partners to ensure quality and holistic care for residents in a post-acute setting. High school diploma is required; one year of long-term care experience is preferred.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.

Responsibilities

• Plan and conduct individual, small and large group activities.
• Maintain communication among staff, residents, families, support personnel, and the public to meet residents' needs.
• Assist in development of monthly activity calendar and attendance records.
• Assist in discharge planning and development of activity care plans and resident assessments.
