Job summary

Jane Pauley Community Health Center in Indiana seeks a School-Based Community Resource Navigator to connect students and families with healthcare, benefits, and community resources.

You will support Medicaid and Marketplace applications, help with Sliding Fee Scale, and guide families through the health system while upholding HIPAA confidentiality. Role requires travel to multiple school sites as needed.

Qualifications

• High School Diploma or Equivalent required.
• Bi-Lingual/Multi-Lingual a plus.
• Two years of outreach navigation with youth/families preferred.
• Customer service experience required.
• Willing to travel 50% to different site locations or schools.
• Valid Indiana Driver's License and proof of auto insurance required.
• Certified Indiana Navigator within 90 days of employment.
• CAC certification within 90 days of employment.
• CHW certification within 6 months of employment.

Responsibilities

• Serve as a resource at the schools and maintain a regular presence for JPCHC clients, families, and staff.
• Participate in outreach events (back-to-school nights, health fairs, screenings, parent meetings, community events).
• Collaborate with school staff and JPCHC departments.
• Assist families with Medicaid, Marketplace, and other public benefits applications.
• Educate students and families on using the healthcare system and government assistance plans.
• Discuss payment options with self-pay families and complete Sliding Fee Scale forms.
• Maintain confidentiality and HIPAA standards in interactions.
• Resolve patient or client issues with providers, plan admins, or insurers.
• Educate on healthcare access and government programs.
• Follow up with families requesting in-person government assistance.
• Assist with food pantries when requested.
• Process Sliding Fee Scale applications.
