Job summary

Adult & Child Health in Indianapolis seeks a School Based Navigator to increase enrollment in comprehensive insurance coverage and to build partnerships for outreach. The role focuses on marketplace coverage and improving access to healthcare through coordination with community resources and services.

The navigator will engage with individuals, families, and employers, manage enrollment processes, and support program goals while upholding privacy and quality standards in a community health

Qualifications

• High school diploma or GED; some college helpful.
• Bachelor’s degree in Social Work, Psychology, Public Health Nursing, Health Education, or related field preferred.
• Two years in community health or related outreach preferred.

Responsibilities

• Help individuals with insurance eligibility options.
• Help employers and employees with insurance eligibility options.
• Facilitate insurance enrollment.
• Provide insurance information in consumer’s best interest.
• Assist interactions with healthcare/social service systems; empower patients.
• Troubleshoot barriers to healthcare adherence.
• Engage community through outreach to recruit uninsured patients.
• Serve as liaison between health/social services and community to improve access.
