Job summary

Jane Pauley Community Health Center, Inc. is seeking a School-Based Community Resource Navigator to connect students and families with healthcare, benefits, and community resources at JPCHC school-based clinics in Indianapolis, Indiana.

The role supports families navigating Medicaid, Marketplace coverage, and other public benefits while promoting system literacy and access to care. This position emphasizes confidentiality, cross-team collaboration with school staff, and outreach through events

Qualifications

• High School Diploma or Equivalent required.
• Two years of outreach navigation experience preferred.
• Customer service experience required.

Responsibilities

• Serve as a school resource, maintaining a regular presence at JPCHC clients, families, and staff.
• Assist families with applications for Medicaid, Marketplace, and other public benefits.
• Educate students and families on using the healthcare system and benefits.
• Maintain HIPAA standards and confidentiality of information.
• Travel up to 50% to different site locations or schools.
