Middlesex Health is seeking an RN Triage/Weekend Coordinator for Homecare. This 16-hour per week role includes every other weekend shifts and entails ensuring patients receive weekend and holiday services in line with hospital policies.

You will process new referrals from all sources and coordinate care with the Home Care team. The ideal candidate is a Connecticut-licensed Registered Nurse with homecare experience, strong multitasking skills, and proficiency with multiple computer systems.

Department:Home Care Clinical Admin

As a Triage RN/Weekend Coordinator you will be responsible to insure that Home Care patients receive all available services on weekends and holidays in accordance with Middlesex Hospital objectives, standards and policies. Processes new referrals from all referrals sources.

Minimum Qualifications:

• A Registered Nurse, who is a graduate of an accredited School of Nursing, currently licensed to practice within the State of Connecticut.
• A non RN licensed clinician such as an Occupational or Physical Therapist may also fill this position with an RN administrator on call.
• All candidates must be computer literate and able to work is multiple systems.
• Must be able to multi-task and may be required to visit patients.
• Homecare experience
• Certification in specialty area preferred.
